Candidates should write their name and address on the back of Bank Draft/Pay Order/IPO and enclose it on the top of the application form in original.
The application should be made in the format specified only. The application should be write by the candidate himself/herself by black pen only (ball point or gel). The photograph pasted should be a color one. The completed application form along with the attested document copies and draft/IPO/pat order should be enclosed in an envelope.
